August 19th, 5-7 Parkview ~ Back to School Night Checklist

VISIT YOUR CHILD’S CLASSROOM…

  • Meet your child’s teacher and drop off school supplies 
  • Verify your child’s transportation plan with your child’s teacher.  
        If it’s incorrect, please go to the office

Forms to Complete

  • Breakfast Options Form - Let us know whether your child may eat breakfast so you won’t incur unnecessary food service charges. Please return this form to your child’s teacher.
  • Nurse - Provide doctor’s orders, allergies, medications, etc to the nurse. 
  • Free and Reduced Lunch Application - Please support our school district and fill out the Free and Reduced Lunch Application even if you don’t think you will qualify. Our funding for our Title I program is directly related to our F/R Rate. If your child is a full-day preschool student, we use the form to determine tuition rates. Those qualifying for Free or Reduced have free tuition! Please return completed forms to the secure box just beside the cafeteria entrance.  If you choose to fill it out at home and send it back later, please place it in an envelope marked w/ATTN: Mrs T. Thanks for your help!

Visit the Cafeteria

  • Car Rider Tags - Every child is required to have a car rider tag. If you already have one from last year or Summer School, it will be the exact same number. You can use your existing cards or get additional copies. Students will receive new plastic tags on their backpacks on the first day. 
  • Bus Barn - You only need to visit this table if the bus pickup or dropoff location shared with you by the teacher is incorrect. Durham can also help verify the location of pickup and dropoff spots and provide approximate times.
  • Food Service - Deposit money in your child’s account, if necessary, and visit about Free & Reduced Lunch.
  • PV Yearbook - Please pick up your child’s yearbook from last year if you already paid. A limited number of extra yearbooks are available for purchase.
  • Parent Square - How to get signed up! (Be sure to download the app)
  • Social Worker - come and meet our social workers and learn more about how they can help your family.
